If you submit a request for a free consultation with Dr. Miller via the website, you will be directed to a HIPAA compliant third-party site for further assistance. On this site personally identifiable information will be collected to fulfill your request and to provide the best service possible. Information collected includes your full name, email address and phone number. A record of your request history is maintained.

In case of any payments for treatment with Dr. Miller, a third-party site is used. Please review the notification on Cookies, Privacy Policy, Terms and Conditions of Use for the respective site. IPNAM Health, LLC will maintain a record of your credit card information as a requirement for treatment.
