Policies for individuals in individual treatment with Dr. Miller.
Insurance
IPNAM Health, LLC currently accepts Medicare. There is a co-pay for patients with Medicare. Dr. Miller does not accept commercial private insurance at this time. However, you may be provided with a detailed invoice to submit to your private commercial insurance company for reimbursement through "out-of-network" coverage. Most insurance companies have a deductible before reimbursement starts. That is a minimum amount you must spend before they will begin to reimburse you. Most insurance companies now cover office-based and video psychiatry visits. Other fee-based services like for telephone calls, secure messaging, and letters typically are not covered by insurance. You are encouraged to check with your insurance company in advance for information on their policies regarding out-of-network visits and reimbursement.
Usual billing codes: 90792 for initial visit; 99212 or 99213 for follow-up visit, 90833 or 90836 for additional psychotherapy .
Fees
Session fees are listed on the Fees page. Payment is required at the start of the session. Payments may be made by Zelle, or using a debit or credit card - American Express, MasterCard, Visa and Discover. Cash and checks are not accepted.
Intake Forms
Once you have scheduled your first appointment with Dr. Miller, you will receive a set of intake forms via the secure portal. These forms include a health history questionnaire, practice and privacy policy information, consent forms, identity verification and credit card information. These forms should be completed and submitted prior to your first appointment.
Late arrivals, cancellations and no-shows
We understand that life happens. Dr. Miller will do her best to accommodate you if you are running late. However, an extension of your session time may not be possible. If you are late to session the full fee is applied and it is possible that your session time will not be extended.
If you are not able to keep your appointment for any reason, please call or send a secure message via the patient portal at least 24 hours in advance of the scheduled visit to avoid a charge. Cancellations with less than 24 hours notice will be considered a no-show and you will be charged the full visit fee. Please consider that insurance companies do not reimburse for no-shows.

If you miss an appointment without notice, Dr. Miller will make 2 outreach calls and send 1 outreach letter to encourage follow-up. If there is concern for your immediate safety, emergency services may be engaged for a safety check as well. After 30 days your case may be closed.
Emergency Services
In the event of a medical or mental health emergency, or other concern for your immediate safety, you should dial 911 or report to your nearest emergency room. If you are having thoughts of suicide dial 988 for the mental health crisis line, or report to your nearest emergency room.
Routine Communications
Telephone calls are returned within 1 business day. Secure messages via the patient portal are returned within 3 business days. Please see the Fees page for more information on guidelines for using these communication systems, and potential charges associated with these services.
Medication prescriptions
General guidelines for prescribing:
The use of medication requires regular monitoring. Documentation of a recent physical exam and certain blood and other tests are needed. Physical assessments and tests are repeated at a frequency dictated by the specific medication. These requirements are for your protection. They help to avoid missing alternative conditions that may have psychiatric manifestations, or that may preclude use of a medication. These steps also help to catch any potential side effects that may arise during treatment. In some cases medications require ongoing monitoring via laboratory studies, other tests (e.g. ECG), and/or physical assessments (e.g. blood pressure, weight). Lack of participation in recommended monitoring may result in discontinuation of the medication.
Active and regular participation in treatment is required to receive a medication refill. If you are attending appointments as scheduled, you should have a sufficient quantity of medications between visits. In the event of a missed appointment, Dr. Miller may consider a bridge refill until the next appointment if it is safe and appropriate to do so. Controlled substances are not refilled without an appointment. The maximum refill amount is 90 days and is offered only for patients who are consistently participating in treatment, and on a stable medication regimen that does not carry refill restrictions.
Medication refills:
Psychedelics, cannabis, and other controlled substances:
Dr. Miller does not offer psychedelic-assisted therapies or medical cannabis prescriptions. Other controlled medications such as stimulant and sedative medications also are not routinely prescribed.
There is active, promising research on psychedelic-assisted therapy and other interventions. If you are interested in exploring psychedelics or other treatments not offered by IPNAM Health, we encourage engaging a reputable academic center.
Informed decision making for complementary treatments
Dr. Miller offers psychiatric services that are considered conventional Western medicine, and those that are complementary. Examples of conventional treatments include prescription psychiatric medications and cognitive behavioral therapy. Complementary treatments include options like meditation and mindfulness and vitamin supplements. In some cases scientific evidence may not be available to the same degree for complementary therapies as you may find for conventional treatments. However, many interventions like mindfulness have years (even centuries) of practical support in addition to a growing body of scientific evidence showing benefits. Dr. Miller emphasizes complementary interventions for which the risk is low and the potential benefits outweigh risks. Dr. Miller is happy to discuss the evidence that guides her recommendations and to share resource information. Complementary treatments are intended to add to and enhance your treatment options, not to dismiss conventional options out of hand. The interventions you choose are up to you.
Ending treatment
Treatment services may end for many reasons. Once you have reached your treatment goals you may decide that treatment is no longer needed. You may relocate or have another change in your circumstances. Or you might decide to pursue treatment with another doctor. Whatever the reason, ideally the end of treatment will be mutually agreed upon and include a plan for a smooth transition of care. An opportunity to say goodbye and reflect on your treatment experience with your doctor may be therapeutic in and of itself.
There are some cases where Dr. Miller may initiate closure of your case. That may occur for reasons including that your clinical psychiatric needs exceed what may be safely provided in Dr. Miller's practice, repeated missed appointments, or lack of participation in treatment. Dr. Miller will provide advance notice in case of this recommendation, and support identifying alternative practices for you to pursue treatment. Once availability of an alternative treatment option is confirmed your case will be formally closed. If you are on medication at the close of treatment Dr. Miller will provide a 30-day refill to support the transition of care as long as it is safe to do so.
